Lifestyle Health Assessments (6 – 1 hour sessions)
Initial session (1-2 hours)
Our initial meeting will involve an assessment covering all major pillars of health (exercise, stress, nutrition, sleep, social connections, risky substance abuse). To optimize your health, we will work to identify categories including pain limitations that need some additional coaching and develop SMART goals together.
Follow-up sessions (1 hour)
These sessions will focus on the categories that need additional coaching and improvement. During these sessions we will help educate on areas for improvement, provide strategies and skills to help you make progress towards your health goals, and provide homework to practice those strategies. For example, if lack of sleep is a primary issue, a detailed action plan will be laid out based on evidence-based research on improving sleep.
We are here to empower and support YOU
You will be scheduled once a month for in person follow up sessions. This time gives you the space to practice your skills and work on concepts between sessions. We strongly encourage biweekly phone/email/Zoom check-ins with your provider to answer any questions that might come up and provide the critical element of accountability.
